Abstract:
In the UK, a large proportion of Foundation doctors are not applying for specialty training at the time when they first become eligible to do so at the end of Foundation Year 2. This behaviour has implications for service delivery now and for the flow of doctors to consultant status. Identifying and examining the drivers for this behaviour is crucial to identify the best ways to deliver and optimise training. Thus, the aim of this PhD was to identify and examine the socio-demographic factors, experiences and preferences which influence Foundation Year 2 doctor (F2) career decision-making and careers behaviour, and to investigate the relative value of these in relation to career intention and behaviour. I used a mixed-method programme of research involving a discrete choice experiment (DCE), a large-scale quantitative survey and multi-site qualitative interviews to achieve these aims. The interviews outlined that a supportive working culture and working conditions, professional development, feeling valued and team support are important to F2 doctors. Based on these findings, Familiarity with Specialty, Culture of Working and Learning Environment, Potential Earnings, Working Conditions and Opportunities for Professional Development were included in the DCE (administered in Scotland in 2016). The DCE identified that geographical location and a supportive working and learning environment were the factors most valued by F2s. The follow-up, UK-wide DCE (2017) plus additional survey questions identified differences between male and female doctors, and those entering medicine as graduates or school leavers, in both careers behaviour and what is valued in a specialty training post. I also found that doctors' intentions at the start of postgraduate training are a good predictor of ultimate career choices. This research provides new knowledge about F2 career decision-making preferences, behaviours and intentions which can inform workforce policy and practice.
