Abstract:
Periodontitis is a chronic inflammatory disease affecting the supporting tissue of the teeth. Previous studies showed that periodontitis has an aetiological association with other systemic diseases, such as cardiovascular diseases, diabetes mellitus and rheumatoid arthritis. Ankylosing spondylitis is a rheumatic disease affecting the axial skeleton and the sacroiliac joints. The concept of a link between ankylosing spondylitis and periodontitis was emerged around 14 years ago due to the common immunopathological picture between these two diseases. This dissertation aimed to investigate the association between ankylosing spondylitis and periodontitis. To save time and cost, self-reported questions were chosen to identify periodontitis. Results from a systematic review showed a high specificity and low sensitivity in using self-reported periodontitis. A series of studies were conducted using different databases to assess the link between these two diseases. The results showed increase in reporting oral health problems among ankylosing spondylitis compared to controls. An association between the presence of ankylosing spondylitis and reporting loose teeth and previous periodontal treatment was present. Increase ankylosing spondylitis disease activity was shown to be associated with reporting loose teeth. In conclusion, AS patients reported high prevalence of self-reported oral conditions relevant to periodontitis. However, the limitations present suggest further investigation.
