Abstract:
Tuberculin skin tests were performed with a range of mycobacterial antigens (the New Tuberculins (Shield et al., 1977) and PPD) on a series of 110 patients with tuberculosis, 7 patients with leprosy and 34 normal controls, all Indonesian subjects. The skin test reactions were measured at 6h, 24h and 48h. Punch biopsies (4mm size) were taken at 48h and the tissue divided, half for formalin fixation and half for cryostat sectioning and immunocytochemical staining with Leu 2a, 3a and M3 monoclonal antibodies to detect CD4 and CD8 lymphocytes and monocyte/macrophages respectively. The inflammatory cells in the dermis were divided into two populations: those still within perivascular foci and those in the intervening dermis and the densities of lymphocyte subsets and monocytes in the intervening dermis were quantitated using techniques described previously (Gibbs et al, 1984). Mast cell densities were estimated on sections stained with Toluidine blue. The distribution and density of the infiltrating CD4 and CD8 lymphocytes and monocyte/macrophages and mast cells were documented for the various subject groups and the different antigens, and comparisons made between them. The histology of differing clinical responses e.g.. early reactions at 6h and the morphologically different 'Koch' and 'Listeria' reactions (Rook et al., 1981) was compared. The histological findings were related to evidence of systemic disease (i.e. levels of acute phase reactant proteins, antimycobacterial antibodies (available from previous reports (Gibson et al., 1987; Hoeppner et al., 1987) and chest X-ray evidence of pulmonary tuberculosis).
